Subject Access Requests
The Practice is registered with the Information Commissioners Office and complies with the General Data Protection Regulation 2018. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the regulation.
Patients who would like access to their records, via a Subject access Request or SAR, can request this ideally using this form below. In each case, please can you specify what is required. This will enable us to process this request quickly and minimise the work required.
Please note that when collecting your SAR you will be required to provide proof of identification.
Access to Records
We encourage as many patients or their legal representatives to have an online account. Patients aged 16 and over can also access to their coded medical record via their online account. Patients who have an online account can request access to records using the options available. Patients can obtain an online account by completing the forms below and bringing them to reception with the required Identification.

If you wish another person, spouse, relative or friend to act on your behalf you can grant permission by completing the form and returning it to the Practice. Please note it is your responsibility to ensure these details are kept up to date.
