Your data and how it can be used

As a Patient at the surgery we want you to make informed decisions about how we share your data. There are a number of choices you as a patient will need to make.

Summary Care Records

Do you want your emergency data available at a hospital if you are admitted in an emergency?

Your ‘Emergency Care’ record allows urgent care providers to see only your current medication and any drug sensitivities and allergies. If you are happy for this to be made available, then you do not have to do anything. If you do not want this record to be made available, please record your preference using the form on this link.

Data Sharing

We use a secure electronic health records system called SystmOne. With your permission, we can share your full records that are held here with other healthcare services that also use SystmOne, who are providing care for you. These other services will ask your permission to view your record.

Many organisations may use SystmOne including GP practices, out of hours services, children’s services, community services and some hospitals. Sharing your health record will help us deliver the best level of care for you. Please note we will need to share you information if you are referred for care outside of the GP surgery.
