Long term conditions are reviewed in the month of your date of birth. This helps us and you plan when you will need to be seen.
Flu vaccination is safe and effective. It’s offered every year through the NHS to help protect people at risk of getting seriously ill from flu.
The NHS website provides information about who can get it, how to book and where to get the vaccine.
These clinics are by appointment and are held by the practice nurses. (Please note that 8 week checks are held by the doctors and the practice nurses, and may be at varying times.)
Please complete our travel risk assessment form at least 6 weeks before you travel. The form will be reviewed by the practice nurse who will check your medical records to identify which of the requested vaccines you need.

It is important to complete the form as early as possible, as an appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine.
Please call the practice one week after submitting your application, to find out what vaccines are required and to book a suitable appointment.
